Initial Consultations are for:

1. New patients to Physio Action
2. New injuries for current clients of Physio Action
3. Clients that have not been seen despite the condition for >3 months

Follow up appointments are for current clients having undertaken an Initial Consultation

If you are going through insurance, you will require the name of your insurer, your membership number and authorisation code at the time of booking.

Please look out for your client form sent after booking as this is essential information required before your appointment – on occasions this may initially drop into your junk folder.

• Women’s Health and Mummy MOT – Only available at David Lloyd, Leeds with Jaime Sawiuk
• Sports Massage – only available at David Lloyd, Leeds and Chapel Allerton clinics
• In Harrogate, all children U16 must be booked in with Jenny Frosztega, our specialist Paediatric Physiotherapist.
